关于扩血管药的应用

有一个问题一直困扰着我:抢救休克时，扩血管药在什么时候什么情况下应用？如何应用比较好？（我的疑惑是:休克时往往血压很低，使用扩血管药会不会加剧血压降低？）敬请解疑，谢谢。

in the reasusation of acute shock pt, vasodilation agents have limited roles. it may be used for cardiac vassal dilation, acute pulmonary hypertension, severe vaso spam which follow acute shock. ideally, vasodilation may be give after fluids is give, and pt's BP is normal. but in the case, you migh want to give nitroglycerin either iv or sublingual for corornary dilation, or CA channel block or inhalation agent for pulmonary hypertension, or beta block for serve tachycardia even pt is in shock.  it is catch 22. all about benefits and risks.  the aforementioned agents are either directly or indirectly cause vasodilation.

抢救休克是要先扩容后再用升压药，扩容要注意病人的HB和HCT,如HCT低于25%就要非常注意，不要过度血液稀释。
