长期肢体神经损伤常导致镜像性疼痛(Mirror-Image Pain,MIP),即损伤肢体对侧的异常疼痛感觉。尽管已经明确部分MIP受中枢伤害感受器调节,但其具体机制仍不明确。前扣带回皮层(anterior cingulate cortex,ACC)是接收来自对侧肢体的外周伤害性信息的关键脑区。本研究中我们建立了雄性小鼠MIP模型,通过单侧坐骨神经慢性收缩性损伤(chronic constrictive injury of the sciatic nerve,CCI)导致双侧后肢的痛觉阈值降低,以及损伤肢体对侧和同侧ACC中表达c-Fos神经元数量增加。使用病毒示踪技术,结果发现部分ACC神经元与对侧ACC神经元形成单突触连接。此外,MIP小鼠中,ACC胼胝体交叉投射神经元中的c-Fos表达增加,表明两个大脑半球ACC神经元之间的交流增强。此外,选择性抑制损伤肢体对侧的ACC胼胝体交叉投射神经元,可使CCI小鼠对侧肢体的伤害感觉恢复正常,而不影响损伤肢体侧伤害感觉的增加。与之相反,抑制损伤肢体对侧的ACC中非胼胝体交叉投射神经元,可使CCI小鼠损伤肢体侧的伤害感觉恢复正常,而不影响未损伤肢体出现MIP。这些结果证明了一个环路机制,即两个大脑半球之间的ACC胼胝体交叉投射导致MIP的发生,并可能促进其他形式的疼痛感觉向对侧转移。
