well, it doesn't matter  big government or small government.  entitlement is alway one of issues hard to manage.  health care is part of entitlement. in usa, medicare is called by politician mediscare. if government puts lots efforts and money into it. then there is always a breaucracy.  eventually the efficacy and fraud will destroy the system. if it is privatized, then, there are always a issue to balance profits and services.  then customers are victims of greed.
