The management of chronic epilepsy in infants and children
has undergone a dramatic shift in recent years. Despite the
development of a variety of specific drug regiments, the
prevalence of pharmacologically intractable seizures is still
high. Furthermore, long administration of anticonvulsant
drugs leads to numerous side effects and neurocognitive
deficits. Advances in neuroimaging and electroencephalography
provide epileptologists with anatomical targets that
mediate some medically intractable seizure disorders.
Recent advances in pediatric neurosurgery have exploited
these technologies and dramatically improved the outcome
in infants and children. However, the physiological and
developmental differences inherent in pediatric patients
present challenges to neurosurgeons and anesthesiologists
alike. The aim of this section is to highlight these agedependent
aspects of the perioperative management of the
pediatric neurosurgical patient.
